Getting started is simple
Request a starter kit by calling
479-271-0277. Your starter kit will provide you with the necessary information and materials to start shipping.
- Fill out a lab script, printable on the bottom of the page, being as detailed as possible. Fold lab script and place in a seperate sealed bag with the impression/model.
- Place cases securely into a box and attach appropriate label.
- Call or email, shipping@gdtlab.com, to request a shipping label. It will be emailed or faxed to you. Then schedule a pick up at your office or deliver your package to the nearest UPS or FedEx location.
- Our turnaround time is 10-14 days depending on your location.
- When your case is finished, you will be notified via email with the tracking number
